It has come to our attention that certain comments have been posted on social media concerning White House Farm Medical Centre.

There is always a GP available on the premises and the comment stating there was "no Doctor" is incorrect.

We have a self service machine available in the reception area for you to record weight and Blood pressure.

Please ask our reception staff for a token to use this.

NHS APP for Covid Pass

A letter with the QR code can be requested by ringing 119.

The Covid pass is on the NHS App. 

If you require the QR code  for your children 12 years and over via the NHS App, they need to have the App in their own name. This means assessment of the child and an appointment with a GP is necessary. Please arrange this well in advance of your planned holiday. Thank you.

We are now offering telephone appointments which can be booked either through online services or the NHS App.

If you are coming to the surgery for a face to face appointment, please attend on your own and wear a face mask. Adults who require physical or emotional support can have one carer or relative attend with them if not having someone present would cause distress or difficulty.

Children can be accompanied by one parent or legal guardian.

Please be aware for second vaccinations there needs to be an interval of 8 weeks before the second vaccine can be given.

We are now able to provide Covid vaccinations for our patients at the surgery.

Complaints

Customer service formWe make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

To pursue a complaint please contact the practice manager who will deal with your concerns appropriately. Further written information is available regarding the complaints procedure from reception.

Extended Access - Appointments outside of Practice Hours

Extra appointments are being provided in the evening and at weekends across Doncaster. The service is an extension of your GP practice and offers routine and same day appointments with a range of clinicians including GP's, Nurses and other Healthcare professionals.

When using this service you may not see your usual doctor, but clinicians from local practices are employed wherever possible.

With your consent the clinicians will be able to access your healthcare record to ensure they can offer the right treatment for you and prescribe medications or refer you for further treatment if necessary.

These appointments are run by Primary Care Doncaster Limited and are offered at the following locations:
