Structured Medication Review (SMR)

• It's an evidence based and comprehensive medicine review taking into consideration all aspects of your health.
• It's a shared decision-making conversation between a patient and a qualified pharmacist to discuss indivudal needs and preferences whilst circumstances are being considered.
• During the review we work with you as equal partners to understand the balance between benefits, risks and available alternative treatments. 

• A review of existing conditions or diagnosing of a new condition.
• Designed to add or start new medications. 
• An examination by a clinician.
• Designed to discuss minor ailments. 

• Safety - balance the benefits and the risks with a healthcare professional.
• Discover the effectiveness of the treatment.
• A presonal approach tailored to your needs.
• Shared decision making.
• Usually take place every 12 months. 

Anyone who has prescribed medication on a repeat basis is entitled to have a structured medication review.

• Prescribed 5 or more medications?
• Experiencing side effects?
• Have any unanswered medication questions? 
• Do you have difficulty taking your medication?
• Are all of your medications in sync to enable easy ordering?

• Do I still need all of these medications?

• What side effects should I be looking out for?

Here at White House Farm the Pharmacy Team carry out consultations over the phone, however if necessary, we can facilitate face to face (please ask upon booking).

SMR consultations are usually 30 minutes. Follow up appointments may be requested within the consultation.
